Lead Case Manager - RHC (5684)

• Provide oversight of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services.
• Conduct intake and individualized needs assessment for clients, develop Individualized Service Plans (ISP) addressing housing goals, income, and personal goals.
• Provide referrals, linkages, and support to resources that help clients achieve their ISP goals, and maintain documentation standards.

This role directly assists individuals experiencing homelessness in breaking the cycle of homelessness by providing case management, referrals, and support to secure housing and improve well-being.

• One year of experience in healthcare, case management, homeless services, or working with vulnerable populations (minimum); three years preferred.
• Analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethical boundaries.
• Valid driver's license, reliable transportation, and ability to qualify for PATH's insurance coverage.
