What you will drive
- Provide oversight of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services.
- Conduct intake and individualized needs assessment for clients, develop Individualized Service Plans (ISP) addressing housing goals, income, and personal goals.
- Provide referrals, linkages, and support to resources that help clients achieve their ISP goals, and maintain documentation standards.

Profile
What makes you a great fit
- One year of experience in healthcare, case management, homeless services, or working with vulnerable populations (minimum); three years preferred.
- Analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethical boundaries.
- Valid driver's license, reliable transportation, and ability to qualify for PATH's insurance coverage.
