What you will drive

  • Conduct daily street outreach to develop relationships and build rapport with clients to assist them with finding a home.
  • Provide street-based case management services connecting clients to long-term supportive services, housing, health, mental health, and other resources.
  • Collaborate with community providers, law enforcement, and landlords to ensure seamless referrals and housing placements.
  • Complete documentation, data entry, and reporting to track client progress and program outcomes.

Profile

What makes you a great fit

  • Two (2) years of experience in outreach case management or a related field.
  • Strong analytical, communication, and problem-solving skills.
  • Ability to work collaboratively in a multidisciplinary team and build relationships with diverse stakeholders.
  • Valid driver's license, reliable transportation, and proof of insurance; ability to qualify for PATH's insurance coverage.
