Metro Line Outreach Case Manager - (5680)

• Conduct daily street outreach to develop relationships and build rapport with clients to assist them with finding a home.
• Provide street-based case management services connecting clients to long-term supportive services, housing, health, mental health, and other resources.
• Collaborate with community providers, law enforcement, and landlords to ensure seamless referrals and housing placements.
• Complete documentation, data entry, and reporting to track client progress and program outcomes.

This role directly helps individuals experiencing homelessness find stable housing and access supportive services, contributing to ending homelessness and promoting social justice.

• Two (2) years of experience in outreach case management or a related field.
• Strong analytical, communication, and problem-solving skills.
• Ability to work collaboratively in a multidisciplinary team and build relationships with diverse stakeholders.
• Valid driver's license, reliable transportation, and proof of insurance; ability to qualify for PATH's insurance coverage.
