Housing Case Manager - North Long Beach (JR 5663)

• Conduct intake and individualized needs assessments for participants, developing Individualized Service Plans (ISP) addressing housing, income, and personal goals.
• Provide referrals, linkages, and support to resources that help participants achieve their ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT) and conduct 90-day reviews.
• Maintain documentation standards, including case file notes and participant files, ensuring confidentiality per HIPAA.

This role directly helps individuals experiencing homelessness in North Long Beach to secure stable housing and improve their well-being, contributing to the broader mission of ending homelessness.

• Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
• Strong analytical, communication, and problem-solving skills.
• Ability to work collaboratively and maintain ethical boundaries.
• Valid driver's license, reliable transportation, and ability to qualify for PATH's insurance coverage.
