What you will drive
- Conduct intake and individualized needs assessments for participants, developing Individualized Service Plans (ISP) addressing housing, income, and personal goals.
- Provide referrals, linkages, and support to community resources to help participants achieve their ISP goals.
- Maintain documentation standards, including timely case file notes and accurate participant files, while ensuring confidentiality per HIPAA.
- Provide crisis intervention and risk assessment services using motivational interviewing and trauma-informed care.
