What you will drive
- Conduct intake and individualized needs assessments for participants, collaborating to develop Individualized Service Plans (ISP) addressing housing, income, and personal goals.
- Provide referrals, linkages, and support to resources that help participants achieve their ISP goals, including medical, mental health, substance use, and financial resources.
- Maintain documentation standards, including timely case file notes and accurate participant files, while ensuring confidentiality per HIPAA.
- Provide crisis intervention and risk assessment services, using motivational interviewing and trauma-informed care to support clients.

- Must have reliable transportation, valid driver's license, and proof of insurance; ability to qualify for PATH's insurance coverage.
