Case Manager - North Long Beach (5678)

• Conduct intake and individualized needs assessments for participants, collaborating to develop Individualized Service Plans (ISP) addressing housing, income, and personal goals.
• Provide referrals, linkages, and support to resources that help participants achieve their ISP goals, including medical, mental health, substance use, and financial resources.
• Maintain documentation standards, including timely case file notes and accurate participant files, while ensuring confidentiality per HIPAA.
• Provide crisis intervention and risk assessment services, using motivational interviewing and trauma-informed care to support clients.

This role directly helps individuals experiencing homelessness break the cycle of homelessness by providing individualized case management, supportive services, and connections to housing and resources, making a positive difference in people's lives.

• Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
• Strong analytical skills, attention to detail, and ability to gather and interpret data.
• Excellent communication, collaboration, and problem-solving skills.
• Must have reliable transportation, valid driver's license, and proof of insurance; ability to qualify for PATH's insurance coverage.
