Case Manager - SPA 7 HSH (JR 5660)

• Conduct intake and individualized needs assessments for participants, collaborating to develop Individualized Service Plans (ISP) addressing housing, income, and personal goals.
• Provide referrals, linkages, and support to resources that help participants achieve their ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT) and evaluate strengths and challenges, conducting 90-day reviews.
• Maintain documentation standards, including timely case file notes and accurate participant files, while ensuring confidentiality per HIPAA.

This role directly helps individuals experiencing homelessness break the cycle by providing case management and connecting them to housing, income, and support services, contributing to PATH's mission of ending homelessness.

• Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
• Strong analytical skills, attention to detail, and ability to interpret data.
• Excellent communication, collaboration, and problem-solving skills.
• Must have a valid driver's license, reliable transportation, and ability to qualify for PATH's insurance coverage.
