Mission

What you will drive

  • Conduct intake and individualized needs assessments for participants, collaborating to develop Individualized Service Plans (ISP) addressing housing, income, and personal goals.
  • Provide referrals, linkages, and support to resources that help participants achieve their ISP goals.
  • Complete program-specific assessments (e.g., VI-SPDAT) and evaluate strengths and challenges, conducting 90-day reviews.
  • Maintain documentation standards, including timely case file notes and accurate participant files, while ensuring confidentiality per HIPAA.
