Mission
What you will drive
- Serve as the primary point of contact for individuals and families seeking information about Medical Aid in Dying (MAiD) in New York.
- Conduct intake calls, assess patient needs, coordinate referrals to participating providers, and provide compassionate patient navigation.
- Maintain accurate and confidential records, manage databases, and prepare monthly reports on program activities.
- Support educational outreach efforts, including conferences, presentations, and community engagements.

Profile
What makes you a great fit
- One or more years of experience in patient navigation, healthcare coordination, hospice, palliative care, social work, counseling, or a similar client-facing healthcare role.
- Bachelorโs degree in education, social work, public health, healthcare administration, or related field (Masterโs preferred).
- Excellent written, verbal, and interpersonal communication skills with empathy and cultural humility.
- Proficiency with Microsoft Office (PowerPoint, Word, Excel), Zoom, and ability to learn Bloomerang CRM.
