Mission

What you will drive

  • Serve as the primary point of contact for individuals and families seeking information about Medical Aid in Dying (MAiD) in New York.
  • Conduct intake calls, assess patient needs, coordinate referrals to participating providers, and provide compassionate patient navigation.
  • Maintain accurate and confidential records, manage databases, and prepare monthly reports on program activities.
  • Support educational outreach efforts, including conferences, presentations, and community engagements.

Profile

What makes you a great fit

  • One or more years of experience in patient navigation, healthcare coordination, hospice, palliative care, social work, counseling, or a similar client-facing healthcare role.
  • Bachelorโ€™s degree in education, social work, public health, healthcare administration, or related field (Masterโ€™s preferred).
  • Excellent written, verbal, and interpersonal communication skills with empathy and cultural humility.
  • Proficiency with Microsoft Office (PowerPoint, Word, Excel), Zoom, and ability to learn Bloomerang CRM.
