Mission
What you will drive
- Conduct intake and individualized needs assessments for clients, developing Individualized Service Plans (ISP) addressing housing, income, and personal goals.
- Provide referrals, linkages, and support to resources that help clients achieve their ISP goals.
- Complete program-specific assessments (e.g., VI-SPDAT) and conduct 90-day reviews.
- Maintain documentation standards, including timely case file notes and accurate client files in compliance with HIPAA.

What makes you a great fit
- Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
- Analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethical boundaries.
- Must have reliable transportation, valid driver's license, and proof of insurance; ability to qualify for PATH's insurance coverage.
- CPR/First Aid training required; must pass tuberculosis test, background screening, and drug test.
