Mission

What you will drive

  • Conduct intake and individualized needs assessments for clients, developing Individualized Service Plans (ISP) addressing housing, income, and personal goals.
  • Provide referrals, linkages, and support to resources that help clients achieve their ISP goals.
  • Complete program-specific assessments (e.g., VI-SPDAT) and conduct 90-day reviews.
  • Maintain documentation standards, including timely case file notes and accurate client files in compliance with HIPAA.

Profile

What makes you a great fit

  • Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
  • Analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethical boundaries.
  • Must have reliable transportation, valid driver's license, and proof of insurance; ability to qualify for PATH's insurance coverage.
  • CPR/First Aid training required; must pass tuberculosis test, background screening, and drug test.
