What you will drive
- Conduct intake and individualized needs assessments for clients, developing Individualized Service Plans (ISP) addressing housing, income, and personal goals.
- Provide referrals, linkages, and support to resources that help clients achieve their ISP goals, including medical, mental health, substance use, financial, vocational, and social support.
- Complete program-specific assessments (e.g., VI-SPDAT) and evaluate strengths and challenges, conducting 90-day reviews.
- Maintain documentation standards, including timely case file notes and accurate client files, ensuring confidentiality per HIPAA.

What makes you a great fit
- Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred.
- Strong analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethical boundaries.
- Must have reliable transportation, valid driver's license, proof of insurance, and ability to qualify for PATH's insurance coverage.
- Must pass tuberculosis test, background screening, and drug test; CPR/First Aid training required.
