• Perform outbound calls to enroll individuals into a food box program or reauthorize them into the program.
• Support patients in making decisions that enhance their healthcare experience.
• Make a positive difference in people's lives through tailored nutrition programs.

This role helps individuals manage chronic illness through medically designed food-as-medicine programs, empowering them to take charge of their health journey.

• High School Diploma or GED required.
• Strong computer skills including data entry, screen navigation, and keyboarding.
• Experience with Microsoft Outlook and Word.
• Excellent customer service, communication, and bilingual skills (English/Spanish).
