What you will drive

Core responsibilities:

  • Conduct intake and individualized needs assessments for participants and collaborate to develop Individualized Service Plans (ISP) addressing housing goals, income maintenance/increase, and other personal goals
  • Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals
  • Complete program-specific assessments (e.g., VI-SPDAT) and evaluate strengths and challenges for addressing short-term and long-term goals
  • Maintain documentation standards as required by the Program and PATH policies, including timely case file notes and complete participant files

  • Collaboration and teamwork skills to work effectively with colleagues, participants, and stakeholders
  • Strong communication skills to convey information clearly and effectively both verbally and in writing
  • Critical thinking and problem-solving abilities to analyze situations, identify key issues, and make logical decisions
  • Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations (preferred)
  • Valid driver's license, reliable transportation, proof of insurance, and ability to qualify for PATH's insurance coverage
