Mission

What you will drive

  • Conduct intake and individualized needs assessments for all clients and collaborate to develop Individualized Service Plans (ISP) that address housing goals, income maintenance/increase, and other personal goals
  • Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals
  • Complete program-specific assessments (e.g., VI-SPDAT) and evaluate strengths and challenges for addressing short-term and long-term goals
  • Maintain documentation standards as required by the Program and PATH policies, including timely case file notes and complete, accurate client files

Profile

What makes you a great fit

  • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field (preferred)
  • Analytical skills and attention to detail, collaboration and teamwork abilities, effective communication skills, creativity and initiative, critical thinking and problem-solving skills, and strong ethics and boundaries
  • Employment eligibility verification, CPR/First Aid training (or willingness to obtain), and ability to pass tuberculosis test, background screening, and drug test
  • Reliable transportation, valid driver's license, proof of insurance and ownership for personal vehicles, and ability to qualify for PATH's insurance coverage
