Per Diem Diabetes Educator Job

• Deliver diabetes education to individuals with diabetes or at risk for diabetes in group or individual settings (virtually or in-person) within New Jersey
• Educate using ADCES accredited DSMES curriculum focusing on ADCES-7, collect self-assessments, and support participants in goal setting and behavior modification
• Document education visits, perform follow-up calls, and participate in reporting data to the Manager as needed
• Promote diabetes-related best practices, develop educational materials, and record educational diabetes self-management videos

This role creates positive change by educating and supporting individuals with diabetes throughout New Jersey, helping them manage their care and potentially lowering diabetes diagnoses through preventative education.

• Licensed Dietitian, Nurse or Pharmacist (Certified Diabetes Educator preferred)
• Spanish Speaking and/or French Creole Speaking a plus
• Diligent, self-motivated, flexible, and able to navigate challenges with strong interpersonal, written and oral communication skills
• Flexibility to work evenings and weekends as needed
