Mission
What you will drive
- Deliver diabetes education to individuals with diabetes or at risk for diabetes in group or individual settings (virtually or in-person) within New Jersey
- Educate using ADCES accredited DSMES curriculum focusing on ADCES-7, collect self-assessments, and support participants in goal setting and behavior modification
- Document education visits, perform follow-up calls, and participate in reporting data to the Manager as needed
- Promote diabetes-related best practices, develop educational materials, and record educational diabetes self-management videos

Profile
What makes you a great fit
- Licensed Dietitian, Nurse or Pharmacist (Certified Diabetes Educator preferred)
- Spanish Speaking and/or French Creole Speaking a plus
- Diligent, self-motivated, flexible, and able to navigate challenges with strong interpersonal, written and oral communication skills
- Flexibility to work evenings and weekends as needed
